General Medication Assessments

A MAC pharmacist can do many things to improve your health and increase your understanding of both your medication and medical conditions. In your first appointment at the MAC, the pharmacist will collect a detailed history regarding current and past medication use to ensure each medication is benefiting you and is safe to continue using. They will also inquire about any concerns or questions that you may have about your health or medications. The following appointments with you are used to discuss the pharmacist’s suggestions as well as to collect further information.

Any recommendations for prescription drug changes are communicated back to your primary care provider. The pharmacist will always discuss any recommendations with you before contacting your primary provider.

Menopause Medication Consultations

Menopause consultation led by a pharmacist educated as a Menopause Society Certified Practitioner and designed to provide education, guidance, and personalized support during the menopause transition. The appointment includes a thorough review of your medical history, an assessment of symptom patterns, severity, and variability, and a discussion of individualized treatment options, including medication therapy when appropriate. If medication initiation or adjustments are recommended, a detailed care plan will be communicated to your primary care provider for prescribing. These sessions are offered both in-person at the MAC clinic or virtually. 

The initial consultation is 60 minutes ($120) and includes a 2-month follow-up phone call with the pharmacist if medications are started. Additional 30-minute follow-up appointments ($60) are available as needed for ongoing support.

Referrals

Patient referrals and self-referrals can be completed online. Patient referrals can also be completed via fax, if desired.
